June 3-August 7 - Forest City Family Practice *Free Athletic Physicals* on Tuesdays & Thursdays after 4:00pm. Appointments must be made by calling the clinic at 265-8840. Physical forms are available at the High School Offices. Any student/athlete under 18 years old must be accompanied by a parent or guardian. If a parent/guardian cannot make the appointment, the student/athlete must have a written permission slip from their parent/guardian in order to receive the physical.
