FREE ATHLETIC PHYISCALS

FOR THE 2009-2010 SCHOOL YEAR

Forest City Family Practice will once again be offering free athletic physicals for the 2009-2010 school year.  All interested students must either have a parent with them at the time of the physical or bring a note giving the practice permission to give the physical.  Students must call ahead to schedule an appointment.  Physical forms may be picked up at all the school offices. 

The dates for the free physicals will begin on June 1^st and run through Aug. 6^th.  Physicals will be given each Tuesday and Thursday only after 4:00pm.
